If you are currently on THA's waitlist, use this form to:
- add or remove household members
- update your contact information
- tell THA if your household includes a person with a disability
- tell THA if your household is currently homeless
- remove your household from the waitlist
- request to be reinstated to the waitlist (if you were notified that you were removed)
You are not required to update THA about changes in household income or employment.
