Community Room Request Form

Which building are you requesting to rent a community room inside?: *

All PPE protocols are in place. Facemasks required.

Organizations

Community Room Request Requirements

  • Any request submitted is dependent on room availability and approval from THA.
  • The requestor must be a legally established, non-profit, city or state organization; or a tenant of Bay Terrace
  • Depending on the usage requested by the organization, THA may require the requesting organization to enter into a Memorandum of Understanding (MOU) or contract for services. The MOU or contract will specify:

               -Services the organization will provide

               -Liability insurance certification provided by the requisition organization

               -Background checks conducted on all staff that will provide the service.

  • The service or event provided must be educational, training, and/or supportive services whether for children or adults.
  • The agency may be a religious organization; the programming offered may not contain any religious, spiritual, or devotional content (e.g. church teachings, religious doctrine).
  • All classes or programs must be open to THA clients although they need not be the sole participants.

  • Social events and family gathering will not be approved for room use.

  • All classes or programs must be open to THA clients although they need not be the sole participants.

Community Room Expectations

  • Place all trash inside the garbage cans. Discard all empty boxes and packaging from your event. If you have food at your event, you MUST empty the trash after your event.
  • Stack all extra chairs 5 high on the back wall
  • Leave room in the standard class room diagram arrangement (please see the diagram posted in each classroom)
  • Leave the classroom clean and neat. Please use supplied dust pan/broom and wipes if needed.
  • Do not move or adjust wall dividers.
  • If you have “Special Needs” (e.g. access to the internet), these needs must be specified on your Community Room Request Form and we will see if we’re able to accommodate your request.
  • Other supplies such as white board markers & erasers, pens, paper, etc. are the responsibility of the requestor.
