By signing below, I acknowledge that I give Mac's Childcare and its entities including Mac's Creative Kids permission to take my child and/or children listed above to onsite and offsite Nature Walks. I also acknowledge that I allow Mac's Childcare and its entities permission to transport my child and/or children to and from field trips, school, home and emergency transport. All drivers will be authorized drivers of Mac's Childcare & Mac's Creative Kids and at least 21 years of age or older. I also acknowledge that this authorization is valid for the entire 2024 calendar year and must be renewed yearly. *
