MEDICAL RELEASE:

I, as the parent or guardian of the above player's name, hereby grant permission to managing personnel or other league representatives to authorize and obtain medical care, at my expense, from any licensed physician, hospital, or medical clinic should the player become ill or injured while participating in league activities away from home or where neither parent nor legal guardian is available to grant authorization for emergency treatment. I assume all risks hazards incidental to my child's participation, including transportation to and from the activities, and do hereby waive, release, absolve, indemnify, and agree to hold harmless the local PONY BASEBALL INC or SOUTHAMPTON SOFTBALL organizations, the CAPRON-DREWRYVILLE ATHLETIC ASSOCIATION, the organizers, sponsors, supervisors, participants, and persons transporting the player to and from the activities, for any and all claims arising out of any injury to the player. CODE OF CONDUCT:

The following rules shall be followed by players, coaches, parents/CDAA members while respresenting CDAA:

1. Players, coaches, and /or parents will not use any profanity during any CDAA sports event,

2. Players, coaches, and /or parents will not engage in inappropriate dialogue, actions, or behavior with umpires, teammates, coaches, opponents, or spectators,

3. Players, coaches, and /or parents will not, under any circumstance, participate in physical violence or threats of physical violence before, during, or after any game or practice,

4. Players, coaches, and /or parents will obey the rules of the game,

5. I will help make this a positie and fun experience for all and report any violations to a head coach, umpire, and/or CDAA board member.

I will honor the CDAA Code of Conduct in my words and actions. I understnd that violators may be asked to immediately leave the premises by a coach, umpire, or CDAA board member at their sole discretion and will be subject to disciplinary action as determined by the CDAA Board of Directors.
