By submitting this application you agree to the submission of your audition for the Lip Sync Battle benefiting Camp Mak-A-Dream. Minors, under the age of 18, must have a parent or legal guardian complete and submit this form on your behalf. In addition to the above, you also give Camp Mak-A-Dream permission to use pictures, video or audio of you/your child in professional, fundraising or marketing materials and activities. Upon submission of this form, your payment will be processed and audition video reviewed. Chosen performers will be notified by telephone the second week of April.
