Westminster College Meal Replacement Order Form FOR QUARANTINE AND ISOLATION STUDENTS ONLY

This order form is for students who have been directed by Student Health Center to quarantine, isolation, or medically recommended isolation. Indicate your food preferences and allergies for all meals while in quarantine/isolation. You may not change preferences. Your meals will be billed to your meal plan or flex plan. Students without a meal plan or flex plan will have their student account billed directly. Three meals will be delivered once a day between 1:00 p.m. and 2:30 p.m. to the building and room identified on the student intake form
