For liability reasons, for the safety of our kids and students @ Family Worship Center, we perform a background check on all persons wishing to serve in the ministries of Family Worship Center. This information will be kept strictly confidential. Information generated from annual background checks will be used only for the purpose of determining the applicant’s suitability for serving in Student and Kids Ministries at Family Worship Center.

I hereby authorize Family Worship Center to conduct a sexual offenders background check once per calendar year through the appropriate national, state, and county agencies and for such agencies to release any information which pertains to any record of convictions contained in its files or in any criminal file maintained on me whether state, local, or national. I hereby release said agency from any liability resulting from disclosure of this information. *
