Complete and submit this form at any time during the current school year if your school district is requesting to place a teacher(s), principal(s), assistant principal(s) or Educational Staff Associate(s) in an assignment for which they do not hold the proper endorsement(s) or certificate. Waivers are for certificated staff that have not completed provisional status and meet one of the following criteria:
- Assigned out-of-endorsement in more than one subject area or out-of-role.
- Assigned in an out-of-endorsement assignment for more than 40% of their day.
