I hereby release the Hoya Junior League Program, its Management Team, coaches, and other officials of the program from any responsibility for injury to my children or self suffered during practices and games, including without limitation transportation to and from practices and/or games. In the event of such injury, I hereby give permission for my child to receive appropriate medical treatment. I agree to abide by the decisions and policies of the Junior League Management Team as in effect from time to time.

I hereby acknowledge and agree that the Hoya Junior League Program, its Management Team and other officials reserve the right to take action against any player, coach and/or fan who demonstrates poor sportsmanship, behavior or is otherwise abusive, confrontational and/or vulgar. I further acknowledge that the Junior League's punishment for such actions may include expulsion from current and future Junior League-sanctioned events.

I hereby acknowledge and agree to the Junior League's 2010-2011 Refund Policy. After October 8, 2010, absolutely no refunds (other than medically documented cases) will be provided.
