I, the undersigned, request that Gregory J. Wastl perform an evaluation that may consist of a health, dietary and lifestyle history, and a scan of my human body-field using the Nutri-Energetics® System. Based on the results, I request that Greg Wastl recommend (1) a dietary and nutritional supplementation program, if indicated, (2) a cleansing and detoxification program, if indicated, and (3) any lifestyle changes as needed, for the purpose of enhancing my health.
I understand that Gregory J. Wastl has significant post-graduate education in dietary analysis, nutritional supplementation therapy, homeopathy, and training in the use of the Nutri-Energetics® System, and that he is a Certified NES Practitioner – Certificate # NES-CP-B-0235.
I understand that Gregory J. Wastl and Nutri-Energetics Systems® (NES) does not diagnose, cure, prevent or treat disease, and is not intended as a substitute for regular medical care. I also understand that if I have a medical condition or concern, I should consult an appropriate healthcare professional. I acknowledge that NES and its claims have not been evaluated by any government agency or regulatory organization.
