Welcome to the online part of your camper's parent packet for the Summer of 2010. Please review the information under the following headings for Parent Handbook Agreement, Equestrian Permission, and Transportation Confirmation to complete your camper's parent packet.

We must have a response in every category in order for your camper's parent packet to be complete. Please note that a completed parent packet is required before your camper can attend Camp this summer.

More than one camper can be added to this form regardless of their session numbers as long as they are
ARRIVING BY THE SAME MODE OF TRANSPORT
and have the
SAME EQUESTRIAN AGREEMENT

You must fill out SEPARATE forms for multiple campers if they are
ARRIVING BY DIFFERENT MODES OF TRANSPORTATION
and/or have
DIFFERENT EQUESTRIAN AGREEMENTS

The paper forms, which you have recieved by mail, and these online signatures are due here at Camp no later than April 1st.

PARENT HANDBOOK AGREEMENT

* By checking the box below I acknowledge (as parent/legal guardian of the above named camper) that I have read and understood The Parent Handbook, and have had the opportunity to ask any clarifying questions of the Camp Augusta Administrative Staff.

EQUESTRIAN PERMISSION SIGNATURE

We must have this agreement on file at Camp if your camper would like to take part in any of our activities involving horses (either a riding or ground clinic).

By typing the words "I agree" in the box below you give permission to allow your camper to participate in the Equestrian Program at Camp Augusta this summer.

If you do not agree, please check the box below signifying your choice.

**If yes, please check with the airline ahead of time regarding information that we will need to board your camper safely on their flight home. Usually we need a permission letter from the parents as well as the name(s) of person(s) picking them up at the airport on your end. (Occassionally a driver's license has been req'd for this.)
