Important Information Before Starting Your Application

The following documents will be required to complete your application. Do not mail transcripts or letters of recommendation to the office.  All uploaded documents must be saved in PDF format:

A). Applicant’s Resume or CV

B). Academic Transcripts of Graduate Studies (unofficial transcripts are accepted)

C). Two (2) letters of recommendation from academic faculty and/or preventive medicine or public health professionals.

*Current students only: a signed letter from the Chair of your department/academic institution 1) certifying your status as an enrolled student in good academic standing and 2) confirming your anticipated graduation date.

D) Personal Essay. In 1,000 words or less you will need to address:

              - Personal interests/career goals and academic or professional experience related to the fellowship

              - Skills/talents you will bring to the fellowship position 

              - Skills/qualifications specifically requested in the fellowship position description

*If you are not a U.S. Citizen: you will be required to attach a copy of your Alien Registration Card (Green Card).
